Kentucky Health Cooperative (KYHC) offers a Disease Management Program that provides education and support to assist you in meeting your health goals.  The program provides you with materials and resources, as well as telephone access to trained Disease Management nurses.

The FREE program is part of your benefits with KYHC.

The program offers:

  • Information on understanding your condition
  • Tips to help you manage your condition and make healthy choices
  • Information on how to avoid risks that can occur with common chronic health issues
  • Tips on how to understand and follow the treatment plan your practitioner has shared with you

Who is eligible?

This program is available to KYHC members who have been diagnosed with one or more of the following conditions:

  • Asthma
  • Diabetes

ATTENTION ALL MEMBERS WHO LIVE IN BULLITT COUNTY!

Below you will find a link (under DIABETES MATERIAL) with information on different classes and workshops the Bullitt County Health Department is offering for the Spring of 2015. Many of the classes offered are FREE. Diabetes 101 for members who have been recently diagnosed with diabetes. A Living Well workshop for members who have other types of chronic conditions like high blood pressure, diabetes, arthritis, cancer and depression. Also, a smoking cessation program.
